JOHNSON/ INSURANCE/ EFFECTED/ ON/ VESSELS/ &/ CARGOES/ Ship Broker & Commission Merchant/ No. 34 BURLING SLIP, NEW YORK./ Vessels Freighted, Chartered, Bought or Sold"; signed on bottom by Johnson and another person (illegible); "DOTY & BERGEN 122 WILLIAM ST. N.Y."
